call our office at (425) 453-1039 to speak with a receptionist and
schedule an appointment.
You should arrive at least 15 minutes prior to your appointment
in order to complete your registration.
To make your first visit a little easier, you can download and
fill out the Patients Registration Form and Patient Health Questionnaire
before you arrive.To download the forms to your computer, click
on each link below. Save each form on your computer and then fill
out and print each one. Make sure to bring the completed forms with
you to your appointment!
